# Health insurance in Germany for Brazilian citizens

4.9/5 · 10,000+ policies since 2009 458 customer responses · [our own survey](/en/review-methodology/)

**Quick answer:** Brazilian citizens can enter Germany visa-free for up to 90 days but still need travel medical insurance with €30,000 cover and repatriation — Brazilian providers (Bradesco Saúde, Allianz Brasil, Porto Seguro) are often not accepted for the post-entry residence permit. For stays over 90 days (residence permit, work, study, family), Care Expatriate (from €58/month) is recognised by every German Ausländerbehörde.

Key facts at a glance Reviewed September 13, 2026

- **€30,000 minimum:** Schengen visa medical cover required by EU Visa Code Art. 15. (Source: EU Reg. 810/2009).
- **~€830/day:** average German hospital cost without cover — the single biggest risk. (Source: Destatis 2024 (indicative)).
- **10,000+ policies:** issued since 2009 for foreigners, students, expats and visitors in Germany. (Source: Care Concept internal data).

## 📌 TL;DR — at a glance

- **Brazilian citizens travelling to Germany need:** either Care Visa Protect (Schengen, up to 92 days, €30,000+ cover) OR Care Expatriate (national D-visa, study, work, Chancenkarte, family — from €58/month).
- **Embassy acceptance:** the Care Concept PDF certificate is accepted by every German Auslandsvertretung — embassy practice can still vary case-by-case.
- **Visa rejection refund:** full premium refund on confirmed embassy rejection before policy start date (check AVB — indicative).
- **After arrival:** GKV starts day 1 of employment; visa-stage cover can be cancelled pro-rata once GKV proof is issued.

Brazilians enjoy visa-free entry to Germany and the entire Schengen Area for up to 90 days in any 180-day period — but the moment you decide to stay longer (work, study, family reunification, marriage, freelance), German law requires recognised health insurance under § 5 AufenthG. The German consulates in São Paulo, Rio de Janeiro, Recife and Curitiba apply the same standards as every other German Auslandsvertretung.

Brazilian private health plans (Bradesco Saúde, Amil, Sul América, Porto Seguro) generally do not satisfy the German Ausländerbehörde — they lack a German postal address, do not document statutory-equivalent benefits, and stop paying after a few months abroad. Care Visa Protect (from €0.85/day) covers the 90-day visa-free window; Care Expatriate (from €58/month) covers everything longer and is the standard recommendation for the Künstlervisa (artist visa), Freelance Visa (Freiberufler, § 21 AufenthG), and family reunification.

## Avoid the mistakes that can delay your application

### Schengen proof missing?

For short Schengen stays, choose insurance that meets the €30,000 minimum-coverage requirement.

### Short stay or longer stay?

Care Visa Protect fits many 1–92 day Schengen visits; Care Economy may fit longer or more flexible stays.

### Wrong insurance can cost time

Before your embassy appointment, check that the proof matches your visa type, dates and destination.

### Multiple entries planned?

If you travel more than once, check the multiple-entry option and the annual day limit before applying.

## One wrong insurance choice can cost you money, time and your application deadline

A medical incident can become expensive fast — but the wrong certificate can also delay your visa, enrollment, residence permit or work start.

🏥

€500–€1,500

Emergency doctor visit

One urgent doctor or emergency-room visit can already create a painful bill — before tests, medication or follow-up treatment are added.

🏨

€2,000–€10,000+

Hospital treatment

If observation, surgery, overnight stay or specialist treatment is needed, costs can quickly move from hundreds to thousands of euros.

🛂

1 wrong certificate

Visa appointment at risk

Wrong dates, missing coverage details, unclear validity area or missing repatriation wording can delay your documents exactly when time matters.

- Wrong or incomplete proof can delay your visa, enrollment or authority process.
- Cheap home-country policies may miss the exact coverage, dates or repatriation wording required.
- The cheapest policy can become expensive if it is the wrong proof for your situation.

Before you apply, check: coverage amount, validity dates, destination area and repatriation cover.

## What German embassies typically check for Brazilian applicants

- Brazilian citizens are the #1 nationality applying for the Freelance Visa (Freiberufler) in Berlin — Care Expatriate Comfort or Premium is the standard recommended insurance.
- Visa-free 90 days starts the day you enter the Schengen Area — overstaying triggers automatic re-entry bans of up to 3 years.
- The Künstlervisa (artist visa, § 21 (5) AufenthG) for musicians, DJs, dancers and visual artists requires recognised insurance for the entire planned stay.
- Brazilian travel insurance (Bradesco, Porto Seguro) is sometimes accepted for the initial 90-day Schengen window but rarely for the residence permit at the Ausländerbehörde.
- Average cost of a single emergency surgery in a German hospital: €8,000–€15,000 — without recognised insurance, you pay out-of-pocket.

Embassy practice can vary case-by-case (Einzelfallprüfung) — not legal or medical advice. Final acceptance always rests with the responsible authority.

## Why act before the visa appointment

Visa appointments can be hard to reschedule. Missing or unsuitable insurance proof can create avoidable delay exactly when time matters.

If your documents are incomplete, you may need extra time to prepare corrected proof.

## More questions Brazilian citizens ask about German health insurance

Is German healthcare free for Brazilian citizens? No. Statutory health insurance (GKV) is funded by income-based contributions (~14.6% + supplementary rate), and private insurance (PKV) by age-rated premiums. Visitors and visa holders must arrive with their own cover — there is no public safety net for uninsured foreigners (indicative, as of 2026).

Krankenkasse in English: "Krankenkasse" is the German term for a statutory health insurance fund (e.g. TK, AOK, Barmer, DAK-Gesundheit). Once a Brazilian citizen signs a German employment contract above the marginal-earnings threshold, statutory membership is mandatory under § 5 SGB V (Einzelfallprüfung applies).

GKV vs PKV for Brazilian citizens: employees under the income threshold (Jahresarbeitsentgeltgrenze 2026 ≈ €73,800) must join GKV. Above the threshold, freelancers and high earners may choose PKV. For the visa stage, private incoming cover (Care Visa Protect / Care Expatriate) bridges the period before GKV starts — embassy practice can vary case-by-case.

How to apply for German health insurance from Brazil: choose a recognised provider (Care Concept policies are issued by HanseMerkur/Advigon in Germany), submit name + date of birth + entry date online, pay by card or SEPA, and receive the PDF certificate by email within minutes — accepted by every German Auslandsvertretung. Not legal advice.

What is the €30,000 minimum? EU Visa Code Art. 15 (Regulation 810/2009) requires Schengen visa applicants to hold travel medical insurance with at least €30,000 cover, valid in all Schengen states, including emergency hospital treatment and repatriation. Care Visa Protect covers €50,000 — well above the legal minimum (Source: EU Reg. 810/2009).

Do Brazilian citizens need health insurance for the Anmeldung (city registration)? Not for the Anmeldung itself, but the Ausländerbehörde requires continuous private or statutory cover for every residence-permit step that follows. Gaps between visa and Anmeldung are the single most common rejection reason — final acceptance always rests with the responsible authority.

Switching from visa cover to GKV after Anmeldung: once a Brazilian citizen signs an employment contract subject to social-security deductions, statutory health insurance starts on day 1 of employment under § 5 SGB V. The visa-stage incoming policy can usually be cancelled mid-term with proof of GKV membership (check your AVB cancellation clause — typical refund is pro-rata, indicative).

What if a Brazilian citizen's visa is rejected? Most reputable incoming insurers — including Care Concept — refund the full premium when a confirmed embassy rejection letter is submitted before the policy start date. Always read the cancellation clause in the AVB before purchase. Refund timing typically takes 5–15 working days (indicative; verify with the insurer).

Family reunification cover for Brazilian citizens: spouses and children joining the resident family member need recognised health insurance from day 1 of entry under § 5 AufenthG. Care Expatriate is commonly used during the bridging period before GKV family insurance starts. The Ausländerbehörde verifies cover at the residence-permit appointment — gaps trigger rejection.

## Brazilian citizens — top questions about German health insurance

Do Brazilian citizens need a visa for Germany?

No — for tourist, business or family visits up to 90 days in any 180-day period. For longer stays (work, study, freelance, family, artist) you need a national D-visa applied for at the German consulate in your home Brazilian state, OR you can enter visa-free and apply for the residence permit (Aufenthaltstitel, § 5 AufenthG) directly at the local Ausländerbehörde after Anmeldung.

Do I need travel insurance for the 90-day visa-free stay in Germany?

Yes — German border officers can require proof of travel medical insurance with €30,000 cover and repatriation. Care Visa Protect (from €0.85/day, minimum €7.50) meets every requirement. Some Brazilian travel insurance (Allianz Travel Brasil, Universal Assistance) is accepted if it shows €30,000 and Schengen-wide validity.

What is the cheapest legitimate insurance for a Brazilian living in Germany long-term?

Care Expatriate Basic (€58/month, ages 13–40) is the cheapest internationally-recognised private cover. Ages 41–60: from €68/month; ages 61–74: from €246/month (Basic, example figures). Once employed in Germany earning under €69,300/year (2025 JVEG), GKV (TK, AOK, Barmer, DAK) is mandatory.

Is Care Expatriate accepted for the Berlin Freelance Visa (Freiberufler)?

For Brazilian artists, musicians, writers, software developers and consultants applying under § 21 AufenthG, it is the standard recommended option. Yes — the Berlin Ausländerbehörde and freelance-visa offices in Munich, Hamburg, Frankfurt and Cologne all accept Care Expatriate (Comfort or Premium variant).

What about the German Künstlervisa (artist visa) for Brazilian musicians/DJs?

The Künstlervisa (§ 21 (5) AufenthG) requires recognised health insurance for the entire planned stay. Care Expatriate Comfort (from €81/month at age 30) is accepted by the Berlin Ausländerbehörde and the Künstlersozialkasse for self-registered artists.

Can I keep my Bradesco Saúde or Amil plan while living in Germany?

The 2026 rate applies from January 1, 2026; check § 13a AufenthV / § 5 SGB V for the current thresholds. Most Brazilian health plans do not pay for treatment in Germany and stop coverage after a few months abroad. The German Ausländerbehörde will not accept them as proof for your residence permit. Switch to Care Expatriate or to German GKV.

How does GKV (statutory) work for Brazilians employed in Germany?

GKV costs ~14.6% of gross salary, split 50/50 with the employer. Once you sign an employment contract in Germany, you are automatically enrolled in GKV (TK, AOK, Barmer, DAK) unless you earn over €69,300/year (2025 JVEG), in which case you may choose private insurance like Care Expatriate.

Are pre-existing conditions like diabetes or hypertension covered for Brazilians?

Care Expatriate excludes pre-existing conditions for the first 6 months unless medically underwritten and approved. Care Visa Protect covers acute, unexpected exacerbations of pre-existing conditions. Declare honestly when applying — non-disclosure voids cover.

Can my Brazilian spouse and children get the same German insurance?

Once one spouse is employed in Germany and on GKV, the non-working spouse and children under 23 are usually free under GKV family insurance (§ 10 SGB V). Yes — Care Expatriate insures each family member individually under the same household contract.

I'm a Brazilian student going to a German university — which insurance?

Under 30: DAK Studenten or TK is mandatory (~€145–€150/month). Over 30 or PhD/exchange students: Care Student (€101.54/month) or Care College (from €27.50/month). For the visa application, Care College (€27.50/month) is the cheapest accepted option.

Does Brazil have a social-security agreement with Germany for healthcare?

Yes — the German-Brazilian social-security agreement (in force since 2013) covers pension rights, but it does NOT cover acute healthcare for Brazilian residents during tourist stays in Germany. You always need private travel insurance for visits.

I'm a Brazilian retiree wanting to live in Germany — what insurance do I need?

Brazilian retirees on § 7 AufenthG residence permit need either Care Expatriate (from €152/month at age 50, €332+/month at age 65) or — if entitled — voluntary GKV. The German-Brazilian totalisation agreement covers pension rights only, not healthcare.
