The three things foreign retirees in Germany typically confuse about health vs long-term care insurance :

1.  1 **They are two separate insurances:** Krankenversicherung pays doctors, hospital, medication, rehab (§ 27 SGB V). Pflegeversicherung pays Pflegegrad 1–5 cash + care-in-kind when daily activities can no longer be done independently (§ 14 SGB XI). One does NOT include the other — and the residence permit only checks for Krankenversicherung. 
2.  2 **Recognised Incoming = acute medical only:** [Care Expatriate from €68/month (41–60) · from €246/month (61–74)](/en/insurance-plans/care-expatriate/overview/) is accepted under § 5 AufenthG for the residence permit and covers doctors, hospital, medication, surgery, repatriation. It does NOT pay Pflegegrad benefits and is NOT a substitute for German Pflegeversicherung. 
3.  3 **Pflegegrad benefits = GKV+SPV or PKV+PPV only:** Cash and care-in-kind under SGB XI are paid only via Soziale Pflegeversicherung (tied to GKV) or private Pflegepflichtversicherung (tied to substitutive PKV). For non-EU retirees who fail KVdR (§ 5 SGB V 9/10), long-term care risk is carried personally while on an Incoming contract.
