Most foreign retirees with residence in Germany do NOT qualify for the statutory KVdR (9/10 rule under § 5 SGB V) and Substitutive PKV is usually unaffordable at older entry age — the realistic recognised lane is Care Expatriate.

Three decision points: GKV vs Substitutive PKV vs recognised Incoming for foreign retirees with residence in Germany :

1. 1 **Check GKV / KVdR eligibility first:** KVdR pensioner cover requires GKV membership for 9 of the last 10 years before retirement (§ 5 Abs. 1 Nr. 11 SGB V). If you do not meet the 9/10 rule, GKV is closed. EU pensioners with a Form S1 (EU Reg. 883/2004) are an exception — but the S1 is not GKV membership.
2. 2 **Substitutive PKV is rarely realistic at older entry age:** German Substitutive PKV underwrites health risk individually; for applicants 60+ with mild pre-existing conditions premiums are typically several hundred euros per month and admission may be refused. Bridge any short gap with [Care Economy from €30 / 30 days (up to 2 years)](/en/insurance-plans/care-economy/overview/).
3. 3 **Recognised Incoming is the realistic long-term lane:** [Care Expatriate from €68/month (41–60) · from €246/month (61–74)](/en/insurance-plans/care-expatriate/overview/) is accepted by every Ausländerbehörde under § 5 AufenthG, up to 5 years per contract, bilingual PDF certificate — one decision for the whole residence period.

## 📌 TL;DR — at a glance

- Foreign retirees moving to Germany rarely qualify for statutory health insurance (GKV / KVdR) — admission requires GKV membership for 9 of the last 10 years before retirement (the so-called 9/10 rule under § 5 Abs. 1 Nr. 11 SGB V). Most newcomers from non-EU countries do not meet this and therefore use a recognised private Incoming product instead.
- EU pensioners with a Form S1 (EU Regulation 883/2004) issued by their home country health-insurance fund are an exception: a German GKV fund administers their treatment locally while the home country bears the cost. This is not the same as German GKV membership and ends if the home pension entitlement ends.
- Realistic long-term lane for the vast majority: Care Expatriate from €68/month at entry age 41–60 and €246/month at entry age 61–74, recognised under § 5 AufenthG, up to 5 years per contract, bilingual PDF certificate. Bridge for the GKV / S1 / embassy wait: Care Economy from €1.00/day (up to age 64) and €2.95/day (age 65 and over), recognised under § 5 AufenthG up to 2 years.

Key facts at a glance Reviewed August 27, 2026

- **§ 5 Abs. 1 Nr. 11 SGB V — KVdR 9/10 rule:** Statutory pensioner cover (KVdR) requires GKV membership for 9 of the last 10 years before retirement. Most newcomers from outside the EU do not meet this and are not admitted to GKV. (Source: Sozialgesetzbuch V § 5 · GKV-Spitzenverband).
- **EU Regulation 883/2004 — Form S1 for EU pensioners:** EU pensioners moving to Germany may transfer cover via Form S1 issued by the home-country fund: a German GKV fund administers treatment locally while the home country bears the cost. Not the same as German GKV membership. (Source: Verordnung (EG) 883/2004 · Deutsche Verbindungsstelle Krankenversicherung – Ausland (DVKA)).
- **Cost of one uninsured day in Germany:** A standard inpatient day in Germany averages €830 (DKG 2024). Care Expatriate from €68/month (41–60) and €246/month (61–74) is the recognised long-term lane under § 5 AufenthG — up to 5 years per contract. (Source: DKG-Krankenhausstatistik 2024 · Statistisches Bundesamt).

## Decision matrix — GKV vs Substitutive PKV vs Incoming for foreign retirees with residence in Germany

**Quick answer:** "When does statutory health insurance (GKV / KVdR) apply for a foreign retiree with residence in Germany — and when is private (Substitutive PKV or Incoming) the typical lane?" — KVdR is admissible only if the 9/10 rule is met (§ 5 SGB V); EU pensioners use Form S1 (EU Reg. 883/2004); for the rest, German Substitutive PKV is risk-rated and often unaffordable at older entry age, so the recognised Incoming product Care Expatriate from €68/month (41–60) or €246/month (61–74) is the realistic long-term lane.

- **1\. GKV / KVdR — only if 9/10 rule is met**

GKV pensioner cover (KVdR) requires GKV membership for 9 of the last 10 years before retirement (§ 5 Abs. 1 Nr. 11 SGB V). Most non-EU newcomers do not meet it and are not admitted to GKV.

- **2\. EU pensioners — Form S1 (EU Regulation 883/2004)**

Apply at the home-country fund before the move; register at a German GKV fund after the Anmeldung. The home country bears the cost. Not German GKV membership; ends with the home pension entitlement.

- **3\. Substitutive PKV — risk-rated, often refused at older entry age**

German Substitutive PKV underwrites health at entry; premiums for applicants aged 60+ with mild pre-existing conditions are typically several hundred euros per month and admission can be refused entirely.

- **4\. Recognised Incoming product — the realistic long-term lane**

Care Expatriate from €68/month (41–60) · €246/month (61–74) — recognised under § 5 AufenthG, up to 5 years per contract, bilingual certificate, no Gesundheitsfragen for standard tier cover.

- **5\. Visitors and embassy / D-visa wait — short bridge**

Care Economy from €1.00/day (up to age 64) · €2.95/day (age 65 and over) — PDF within minutes, recognised under § 5 AufenthG up to 2 years; switchable to Care Expatriate when the long-term permit is granted.

- **6\. Schengen pre-visit before the residence application**

Care Visa Protect from €0.85/day (up to age 64) · €2.60/day (65–74) — EU 810/2009 compliant, repatriation included; ends with the Schengen visa and does not extend into a residence permit.

- **7\. Pre-existing conditions — Incoming exclusion (§ 19 VVG)**

Incoming products exclude pre-existing chronic conditions per AVB. Disclose at sign-up; for chronic-care needs from day 1, German Substitutive PKV or KVdR (where eligible) is required.

- **8\. Family — sponsor's GKV Familienversicherung (§ 10 SGB V)**

Spouses can join the sponsor's GKV Familienversicherung only if the sponsor is GKV-insured and the spouse's own income is below the threshold. PKV/self-employed sponsors → spouse needs an own contract.

- **9\. Care insurance (Pflegeversicherung) follows health cover**

GKV pensioners pay Pflegeversicherung as part of the GKV contribution. Substitutive PKV requires a parallel private Pflichtpflege contract; Incoming products do not contain a Pflichtpflege component.

- **10\. Switching back to GKV later**

After 55, switching from PKV / Incoming back to GKV is normally blocked (§ 6 Abs. 3a SGB V). Plan the long-term lane carefully; Care Expatriate up to 5 years per contract is renewable for the typical residence period.

## The three recognised insurance lanes once GKV/KVdR is closed

Lane 1 / 3

### Recognised long-term lane — Care Expatriate (primary)

Care Expatriate from €68/month at entry age 41–60 and €246/month at entry age 61–74. The realistic long-term lane for foreign retirees who fail the GKV 9/10 rule and for whom Substitutive PKV is unaffordable. Up to 5 years per contract; bilingual PDF certificate; accepted under § 5 AufenthG.

**Need:** Care Expatriate · ≤ 5 years · § 5 AufenthG

Lane 2 / 3

### Bridge / GKV / S1 / D-visa wait (1 day – 2 years)

Care Economy: per-day pricing by age — from €1.00/day for ages up to 64 (with deductible) and from €2.95/day for ages 65 and over (with deductible), minimum €10 premium. Bridges the GKV / S1 administration window and is recognised under § 5 AufenthG up to 2 years.

**Need:** Care Economy · ≤ 2 years · § 5 AufenthG

Lane 3 / 3

### Schengen pre-visit before the residence application (≤ 90 days)

Care Visa Protect: from €0.85/day (up to age 64) and €2.60/day (65–74), minimum €8.50 premium. Sum insured well above the €30,000 EU 810/2009 minimum, repatriation included, valid in all Schengen states. Ends with the Schengen visa.

**Need:** Care Visa Protect · ≤ 92 days · EU 810/2009

## Avoid the mistakes that can delay your application

### Senior prices can surprise you

Age matters. For seniors, rates can be much higher, so show age-based pricing before the visitor applies.

### Short visit or long stay?

Care Visa Protect or Care Economy may fit visits; Care Expatriate may fit selected longer family-stay cases.

### Entry age limit matters

Many options have an entry-age limit. Check eligibility before preparing visa or residence documents.

### Coverage gaps create stress

Choose the coverage period carefully if travel dates, family visit length or residence timing are uncertain.

## GKV/KVdR closed and uninsured — €830/day hospital risk for foreign retirees in Germany

## One wrong insurance choice can cost you money, time and your application deadline

A medical incident can become expensive fast — but the wrong certificate can also delay your visa, enrollment, residence permit or work start.

🏥

€500–€1,500

Emergency doctor visit

One urgent doctor or emergency-room visit can already create a painful bill — before tests, medication or follow-up treatment are added.

🏨

€2,000–€10,000+

Hospital treatment

If observation, surgery, overnight stay or specialist treatment is needed, costs can quickly move from hundreds to thousands of euros.

👴

Age changes everything

Senior pricing and eligibility matter

Older parents or seniors may face higher prices and entry-age limits, so the wrong choice can create surprises before applying.

- Wrong or incomplete proof can delay your visa, enrollment or authority process.
- Cheap home-country policies may miss the exact coverage, dates or repatriation wording required.
- The cheapest policy can become expensive if it is the wrong proof for your situation.

Before you apply, check: coverage amount, validity dates, destination area and repatriation cover.

## GKV said no, Substitutive PKV said hundreds of euros — Care Expatriate is the recognised lane that says yes

## Why act before family visit or residence paperwork

Older visitors often face age-based pricing and eligibility limits. Waiting too long can make the right option harder to choose.

👴

Age affects price

Senior rates can be much higher, so check pricing before preparing documents.

📄

Entry-age limits matter

Many incoming insurance options are only available up to a specific entry age.

⚡

Short visit or long stay?

Care Visa Protect, Care Economy and Care Expatriate serve different stay lengths.

✅

Avoid coverage gaps

Choose the coverage period carefully if family visit dates or residence timing are uncertain.

## FAQ — Statutory vs private health insurance for foreign retirees with residence in Germany

## Frequently Asked Questions

When do retirees qualify for German GKV instead of private?

Only if the KVdR 9/10 rule is met: 90% of the second half of working life must be in German GKV under § 5(1) 11 SGB V. Foreign retirees rarely qualify. Default is Care Expatriate from €246/month (age 61–74).

Is voluntary GKV possible for foreign retirees residing in Germany?

Yes, if the Vorversicherungszeit (§ 9 SGB V, 24 of last 60 months in GKV) is met. Contribution ~14.6% + 1.7% Zusatzbeitrag on worldwide pension income — often reaching €700–900/month. Care Expatriate at €246/month is usually cheaper.

Which pricing applies for private cover of retiree residents?

Care Expatriate age 61–74: €246/month (Basic, SB 150), €322/month (Comfort, SB 150) and €432/month (Premium, SB 0). All tiers cover up to 60 months and are accepted under § 5 AufenthG.

Can EU-pensioners use the S1 form for German GKV?

Yes — eU Regulation 883/2004 lets EU pensioners register their home S1 with a German Krankenkasse — cover is provided by GKV, cost carried by the home country. Non-EU retirees use Care Expatriate from €246/month.

How does the Ausländerbehörde treat retiree cover for residence permits?

Any GKV Mitgliedsbescheinigung or Care Expatriate PDF (from €246/month, age 61–74) is accepted under § 5 AufenthG for § 7 general permits. Travel or Schengen policies are rejected for stays over 90 days.

What is the difference between public and private health insurance for retirees in Germany?

Public insurance (GKV) calculates premiums based on income, while private insurance (PKV) bases them on age and health. GKV offers family co-insurance for spouses; PKV does not. For foreign retirees, GKV access requires sufficient pre-insurance periods. [Care Expatriate from 58 €/month](/en/insurance-plans/care-expatriate/overview/) serves as a bridge solution until status is clarified.

Can I get public health insurance in Germany without German pension contributions?

Without German pension contributions, accessing the KVdR (statutory pension insurance) is very difficult. You may apply for voluntary GKV membership, but approval depends on prior insurance history. EU citizens can use the S1 form. Meanwhile, [Care Expatriate](/en/insurance-plans/care-expatriate/overview/) provides immediate coverage from 58 €/month.

How much does health insurance cost for foreign retirees living in Germany?

Costs vary significantly. GKV charges ~14.6% of income; PKV premiums depend on age and health (often 300–600 €/month for retirees 65+). As a bridge solution, [Care Expatriate starts from 58 €/month](/en/insurance-plans/care-expatriate/overview/) and is accepted by the foreigners office for residence permits.
