What information must a health-insurance certificate contain for a German embassy? 

For Schengen visas, the certificate should also show medical repatriation and at least €30,000 medical cover. The core fields are passport-matching name, validity period, medical cover amount, territorial scope, insurer details and policy number.

Is a PDF printout enough for the German visa appointment? 

810/2009 Art. German embassies verify insurance certificates under EU Reg. 15 within 3–15 working days. Often yes, but format rules differ by consulate. Check the website of your German mission before the appointment and bring both a printed PDF and a digital copy.
