# Germany's Healthcare System: Statutory vs Private Cover for Foreigners 2026

Sources: § 5 SGB V · § 257 SGB V · JAEG 2026 (€77,400) · § 5 AufenthG · BAMF · HanseMerkur AVB · Care Concept AG

**In Germany, health insurance is mandatory by law (§ 193 VVG) — no exceptions for foreigners.** The country runs a dual system: _statutory_ insurance (GKV) covers roughly 90% of residents and is mandatory for most employees and students under 30, while _private_ insurance (PKV and private Incoming products) is the path for high earners, freelancers, retirees and most short- to mid-term foreigners on a residence permit.

For international students the standard private path is [Care College from €27.50/month](/en/insurance-plans/care-college/overview/) (12–40 years, 1–60 months, recognised by German universities). Freelancers, retirees and residence-permit holders typically choose [Care Expatriate from €58/month](/en/insurance-plans/care-expatriate/overview/) (up to 5 years). Visitors, family-visit and Chancenkarte stays run on [Care Economy from €30 / 30 days](/en/insurance-plans/care-economy/overview/) (up to 2 years).

Below: who can join GKV (and who cannot), what GKV costs in 2026, when private Incoming is the cheaper or only legal path, what doctors, hospitals and pharmacies typically charge without cover, and how to obtain a recognised certificate within 10 minutes online.

Key facts at a glance Reviewed August 21, 2026

- **Statutory (GKV) — who is in by law:** employees earning up to €77,400 gross per year (JAEG 2026), students enrolled at a German university below age 30, registered family members, and recipients of certain social benefits — § 5 SGB V. Contribution: 14.6% of gross income (split 50/50 between employer and employee) plus the average 1.7% Zusatzbeitrag. (Source: § 5 SGB V · JAEG 2026).
- **Private (PKV / Incoming) — who can or must choose it:** freelancers and self-employed, employees earning above €77,400/year, students aged 30+ or after the 14th semester, retirees without a German pension entitlement, and many foreigners on a temporary residence permit who are not eligible for GKV. Care Expatriate from €58/month is the standard private Incoming tariff for long-stay residents. (Source: § 6 SGB V · HanseMerkur AVB).
- **Real out-of-pocket cost without insurance:** GP visit typically €40–80 (private rate), specialist €80–200, ER €200–500, hospital day €600–1,200, MRI €350–600 (GOÄ factor 2.3× to 3.5×, indicative ranges). A single uninsured day in a German hospital can exceed an entire month of Care Expatriate or Care College. (Source: GOÄ private fee schedule (indicative)).
- **Residence permit & Ausländerbehörde:** § 5 AufenthG requires uninterrupted recognised health cover for the entire residence-permit term. Both GKV and recognised private Incoming (Care College, Care Expatriate, Care Economy) typically satisfy this requirement; missing or expired cover is a common refusal reason. (Source: § 5 AufenthG · BAMF).

## The four mistakes foreigners make when choosing between GKV and private cover

**Quick answer:** "Should I just take the cheapest GKV?" — Not always. If you are over 30, a freelancer, on a short residence permit, or earning above €77,400/year, GKV is either closed to you or far more expensive than a recognised private Incoming tariff. The four pain points below cause most of the wasted money and rejected residence-permit files.

## Avoid the mistakes that can delay your application

### Visitor insurance may be too short

For multi-month or multi-year stays, Care Expatriate can be a better fit than short visitor coverage.

### Statutory or private?

Freelancers, self-employed people and some incoming long-stay cases may need private incoming coverage instead of German statutory insurance.

### Residence proof requested?

Care Expatriate can provide PDF confirmation after successful application for visa or immigration documents.

### Renewal stress later

A longer coverage term can reduce repeated renewal pressure during projects, residence processes or long stays.

## Reality check: what one uninsured week in Germany really costs

## One wrong insurance choice can cost you money, time and your application deadline

A medical incident can become expensive fast — but the wrong certificate can also delay your visa, enrollment, residence permit or work start.

🏥

€500–€1,500

Emergency doctor visit

One urgent doctor or emergency-room visit can already create a painful bill — before tests, medication or follow-up treatment are added.

🏨

€2,000–€10,000+

Hospital treatment

If observation, surgery, overnight stay or specialist treatment is needed, costs can quickly move from hundreds to thousands of euros.

🧳

Up to 5 years

Short visitor cover may be too weak

For long stays, freelance work or residence documents, short visitor insurance may be too short or not the right proof.

- Wrong or incomplete proof can delay your visa, enrollment or authority process.
- Cheap home-country policies may miss the exact coverage, dates or repatriation wording required.
- The cheapest policy can become expensive if it is the wrong proof for your situation.

Before you apply, check: coverage amount, validity dates, destination area and repatriation cover.

## Why act before your residence permit, enrolment or job-start date

## Why act before your residence documents are due

Long-stay proof can become urgent during visa, residence permit, project or relocation steps. Short visitor cover may not be enough.

🧳

Long stay, different proof

Care Expatriate can fit longer incoming stays up to 5 years, depending on age and selected plan.

📄

Residence documents need clarity

Your proof should match destination, coverage period and long-stay purpose.

⚡

Do not wait for renewal stress

Preparing longer coverage early can reduce repeated extension pressure.

✅

Private or statutory?

Freelancers, self-employed people and employees on assignment without German statutory insurance may need a different route than employees.

## Get a recognised certificate in 3 steps

10 minutes online. PDF certificate issued instantly — upload directly to your university enrolment, residence-permit file or embassy application.

## Frequently Asked Questions

Is health insurance mandatory for international students in Germany?

Yes. Without proof of insurance (Versicherungsnachweis), enrolment at a German university is not possible — it is a legal requirement for every foreign student regardless of nationality. Students under 30 typically join statutory GKV (e.g. DAK-Gesundheit), which sends the M10 electronic notification universities need for enrolment. Students over 30 or GKV-exempt use Care Student (€101.54/month), recognised by universities as GKV-equivalent.

What is the cheapest health insurance for foreign students in Germany?

It is not valid for university M10 enrolment; degree students under 30 use statutory GKV and over-30 or exempt students use Care Student. Care College is the cheapest recognised route for language schools, Studienkolleg and DSH preparation — from €27.50/month for ages up to 29 (Basic tier, 1–24 months) with an instant PDF certificate accepted by German embassies and the Ausländerbehörde for the visa and residence permit.

Can I get student health insurance in Germany if I am over 30?

Yes. Students over 30 cannot join the standard GKV student tariff (age limit under § 5 SGB V) but Care Student at €101.54/month (including care and pension contributions) is recognised by German universities as GKV-equivalent for enrolment. The PDF certificate is accepted for the university registration and the residence permit — case-by-case review by the university; not legal advice.

What does GKV cost per month in Germany?

17.8% of gross salary in 2026, split equally between employer and employee (~8.9% each). At €50,000 gross that is €741.67/month total, €370.83 borne by the employee. Family members (spouse, children) are covered free under § 10 SGB V.

Who must switch from statutory to private insurance in Germany?

No one is forced — employees above €77,400/year gross (JAEG 2026) may opt into PKV under § 6 SGB V. Freelancers under § 21 AufenthG typically use Care Expatriate from €58/month because voluntary GKV re-entry is closed for most new arrivals.
