Two narrow exceptions exist: EU citizens with a home-country state pension may use the S1 portable form under EU Reg. 883/2004 to keep statutory cover while living in Germany; spouses of an active GKV member may join via Familienversicherung (§ 10 SGB V) when own monthly income stays under €535. Outside those two paths, the long-term answer is Care Expatriate up to 5 years or, for short visits before the decision, Care Economy from €30 / 30 days.
